I now have about 4 GB in the old versions directory. Am I supposewd to clean this up manually or is there a way to have this automated?

Wow, that's a lot! 
Yes, you can clean this automatically if you like. Type
tms config
from the command line or press the "Configure" button in tmsgui, or just open tms.config.yaml in a text editor.
Search for versions to keep and set the number to the one you prefer. (0 means no current or old downloads at all, 1 would keep the current but not old ones).
Next time you run smartsetup it should cleanup the folder. If you want to do it now, just type
tms build
